Earlobe keloid is the most common complication following ear piercing. It usually develops after an episode of wound or skin infection. The keloid grows in size, becomes hard and sometimes symptomatic with itchiness and tenderness. It does not go away on it’s own. The recommended approach to the treatment of earlobe keloid is usually a combination of surgery and micro-injections thereafter to avoid a recurrence.
Surgical excision is the first step to removing the hard nodular keloid lump. This is performed by our experienced specialist surgeon under local anesthesia. As much of the keloid is removed as possible and the wound is closed with much distortion to the earlobe. A clean surgical excision ensures optimal healing and minimizes the risk of recurrence. Union of the wound is achieved after 1 week when the stitches are removed.
One month after excision when the wound is fully healed and stable, we will begin the second step with intralesional micro-injections. We advise gentle massage of the scar with anti-scar gel at home between injections. Serial intralesional anti-scar micro-injections are done every 4-6 weeks until the scar remains soft and pliable. Usually, most of them can be controlled after 3-5 treatments. There is a 10-20% risk of recurrence of the earlobe keloid.
